The Hippokrates Programme in Spain

Why in Spain?
Spain has one of the oldest GP programmes in Europe and has come through important developments in Family Medicine in the last few years. Spanish is a worldwide spread language that attracts many people to get to know it. Furthermore, Spanish people and our outgoing culture pull toward millions of tourists to visit our country.

How is Family Medicine Training in Spain?
Since 1978 until 2005 it was a three years vocational postgraduate training programme. It is compulsory in Spain to work as a GP in the public system (after the EU Directive from 1995). From 2005 it became a four years programme, with the advantages that this has. The fourth year fully takes place in a teaching surgery under the supervision of an accredited tutor.
The teaching in GP in Spain is based on what we call Docent Units, everyone with its own coordinator, which controls and supervises the resident's learning and skills, also reviewing the quality of teaching of every tutor in the Unit. The Units not only run the resident's education, but also the sanitary personnel that work in its area of Health, by offering courses, giving advice on research, etc.
Except for some few universities in Spain, Family Medicine is not very much integrated in the pregraduation curricula, so in general there is not much involvement of GPs at University teaching. We are improving in this matter though.

How is the Health System in Spain?
IIt is a public universal system with open access to almost all of the services. It is also based on the idea of Primary Care Services acting on a gate-keeping role.
The population receives all services with "no money in the middle", as all services are covered through national central taxes paid from workers on their wages and pensions.

How to run the Hippokrates programme in Spain?
The idea is to have the visitors in some of the excellent health centres in many places, including Barcelona, where the central office of the Spanish Society of Family and Community Medicine (SemFYC- member of WONCA) is established. SemFYC has the leadership in Spain in all issues related to research and CME activities for family physicians and residents. SemFYC runs every year the largest GP Conference in Europe with some 4500 delegates.

Introduction, Location
Our Primary Health Care Centre, (Centre d’Atenció Primària- CAP) La Marina, is placed at one of the outskirt districts of Barcelona just a few minutes from the Olympic area with the Montjuïc mountain, situated at east, Gran via de les Corts Catalanes at north and the coast to the west.

 

How to arrive? It has an easy access by public bus (no. 9, 37, 79,109, from Plaça Espanya metro station) or Magòria station by FGC (Ferrocarrils de la Generalitat de Catalunya) from Plaça Espanya also. By train it is possible to arrive to Sants station and then take bus number 109.In the future (2008?) by Line number 9 of metro.


Our population is mainly medium-low income social class. Industrial estates have been replaced by more than 10.000 new apartments, in such a way that it has been an important influx of patients to the health centre. The population in the area mainly speak Spanish more than Catalan, and in the last years the immigrant population has increased, especially from Morocco and South America.

We have no local hospitals nearby resulting in an increasing workload.

Work force, Activities (teaching and research)
In our centre there are two working turns: morning (8am- 3pm) and afternoon (2pm-8 pm). There are: 12 family physicians, 4 paediatricians, 1 dentist and 14 nurses (8 paediatrics nurses) and 2 nurses assistant. There are also 10 administrative staff and a social worker. Others specialists work some days in the centre: rheumatologist, endocrinologist, neurologist, psychiatrist, psychologist, pulmonologist and cardiologist. Some complementary medical tests are performed in our centre: spirometry, doppler ultrasound, blood test, electrocardiogram, pulse oxymetry.

 

Our CAP La Marina has been accredited as a teaching centre since 1996, we have had more than 60 family doctors in training, and train 11 medical students and 26 nurse students every year.  We have a professor from the University of Barcelona and we give doctors everything they need to do the doctoral thesis.

 

There’s a big research activity in the centre and it is one of the centres with most scientific production of Barcelona and Catalonia. There are solid lines of research: cardiovascular risk, Diabetes mellitus, Helicobacter pylori infections, mental disorders, on which we have received several awards and produced lots of publications. We also do community activities on health–related issues.

 

Two clinical meetings are taking place daily, early in the morning and late in the evening. A third session at 2 pm is shared by both turns. Different related medical sessions are programmed .We have a very friendly and familiar atmosphere among all professionals.


Most of GPs would be very open to the possibility of a foreign colleague to visit us in our practices. Most of GPs have knowledge of written English although only some can speak it. It is therefore desirable that our visitors can speak Spanish or French (second language of most GPs in our centre).

It would also be desirable if the visit would not be around Christmas time or during the summer period avoiding GPs holiday terms.
We will try to facilitate accommodation at a cheap rate and perhaps in some periods particular accommodation might be possible.

INTRODUCTION
First of all we would like to welcome you to the Hippokrates Program in Spain.

The Las Vegas Primary Health Care Centre is located at the outskirst of Avilés, the third most important city in Asturias.

The age of the population in this area includes both young couples starting their lives and elderly people. Most of people have a low-medium income and work in the nearby factories and others in agriculture.

This area is a quiet half urban-half rural neighbourhood and is well connected to the city centre by bus and also by train.

ACTIVITIES
Our working day goes from 8am until 3pm from Monday to Friday and we both do consultations in the centre and home visits. The Health Centre continues open until 10pm on working days and also during the weekends to attend emergencies.

The Health Centre staff includes 6 family physicians, 2 paediatricians, 1 dentist, 8 nurses, 3 nurses assistants, 1 physiotherapist, 1 midwife, 1 social worker, 5 administratives and 1 attendant. This multidisciplinary team allows us to attend many different aspects of the Primary Health Care. The Centre is equipped with several basic tools such as blood and urine tests, electrocardiogram, spirometry, pulse oxymetry, doppler ultrasound, Wood lamp, etc.

Two clinical meetings are held every Wednesday and Friday morning to discuss medical protocols, clinical cases and bibliographic reviews.

Our Centre was accredited as a training centre for Family Phisician residents in 1998. Ever since then we have been training two or three residents each year and according to the final opinion questionary they are very satisfied with the Centre training skills.

In order to offer you the best attention we would prefer having only one visitor at a time. We would be delighted to show you our daily routine and our multidisciplinary work. We would also do our best to help you find accommodation either in a hotel or staying with a family.

Location
Our Primary Health Care Centre, (Centro de Salud Vélez Sur), is located in the city of Vélez Malaga in the outskirts of Malaga city, district of Axarquía, just a few minutes from Malaga city.
How to arrive? Most of our visitors come from Malaga city. They take the bus “ALSINA-GRAELLS” to Vélez Málaga from the bus station in Malaga city or in the Muelle Heredia in front of the Marina Square. Alsina Graells telephone number in Málaga is 0034952318995 / 0034952341738 or 0034952501731 in Vélez Málaga. This bus takes you directly to the city of Velez Malaga, where our health centre is located.

Our population is mainly from a medium-low income social class. Big areas are business and tourist. Population in that area mainly speaks Spanish and over the last years immigrant population has been increasing, especially from Africa, South America and East Europe. We refer to a 3rd level hospital, and in our primary care health centre we have a Mental Health Centre District.

Activities (teaching and research): In our centre there are two working shifts: morning (8am- 3pm) and afternoon (1pm-8 pm). We are: 16 family physicians, 3 paediatricians, 1 dentist and 13 nurses (1 paediatrics nurse) and 2 assisting nurses. There are also 6 administrative staff members and a social worker. In our primary health care centre there is an emergency care setting 24 hours a day every day of the year. Some complementary medical tests are carried out in our centre: spirometry, doppler ultrasound, blood test, electrocardiogram, pulse oxymetry, digital retinography.

Vélez Sur was accredited as a teaching centre in 2005, we have more than 4 family doctors in training, and a mean of 5 nurse assistant nurses every year. We are currently involved in several multicenter research studies, in collaboration with the “Unidad Docente de Medicina Familiar y Comunitaria de Málaga”.

In our centre, there are 4 solid lines of investigation: Cardiovascular risk, Diabetes mellitus, COPD, Smoking cessation programme. Two clinical meetings take place daily, early in the morning and late in the evening. A third session at 2 pm is shared by both shifts. Many medical sessions are scheduled. We have a very friendly and familiar atmosphere among all the professionals.

Most GPs are very willing to receive a foreign visitor in our practice. Most GPs understand written English although only some of them can speak it. It is therefore desirable that our visitors can speak Spanish. It would also be desirable if the visit was not neither in Christmas nor in summer time.

We will try to facilitate accommodation at a cheap rate and perhaps in some dates a particular place for accommodation might be possible.
